AZ&Me™ Prescription Savings program for healthcare facilities

What is it?

The AZ&Me™ Prescription Savings program for healthcare facilities is designed to help provide AstraZeneca medicines to low-income patients through qualifying facilities such as: 

  • Disproportionate share hospitals 
  • Community health centers 
  • Community free clinics

Program highlights

  • Bulk replacement program based on a facility's qualifying product utilization 
  • Qualifying facility outpatients can receive their AstraZeneca medicines free of charge or for a nominal, facility-assessed dispensing fee from the qualifying facility outpatient pharmacy

Facility Eligibility

Facility qualification criteria includes, but is not limited to:

  • Existence of an on-site, licensed out-patient pharmacy or dispensary 
  • Tax-exempt status 
  • Policies and procedures ensuring that the appropriate controls and security mechanisms are in place to ensure that the facility is compliant with AstraZeneca requirements 
  • Facility patient eligibility criteria that, at a minimum, meets AstraZeneca guidelines for patient eligibility, including patient income thresholds, lack of patient prescription insurance and annual patient eligibility review

The AZ&Me™ Prescription Savings program for healthcare facilities is open for facility enrollment. If you would like to be considered for enrollment in the program, please call 1-866-325-8198 to request an application kit. Please note that enrollment in AZ&Me™ Prescription Savings program for healthcare facilities is limited based on facility eligibility and program availability.
