AZ&Me™ Prescription Savings program for people with Medicare Part D

The AZ&Me™ Prescription Savings program for people with Medicare Part D is designed to provide significant savings to qualifying people enrolled in Medicare Part D prescription drug coverage plans but are having difficulty affording their AstraZeneca medicines.

Are you eligible?
If you are enrolled in Medicare Part D, you may be eligible for the program if you meet the following criteria:
- You are an individual with an annual income below $30,000 or a couple earning less than $40,000 per year
- You are taking an eligible AstraZeneca medicine
- You have spent at least 3% of your annual household income on prescription drugs this year
Program cost structure
Upon completion of the documentation requirements and full enrollment, you will have a choice of getting a 30-, 60-, or 90-day prescription filled at your local pharmacy (if prescribed by your health care professional), which could mean additional savings on the 60- and 90-day fills.
Individuals with annual incomes at or below $15,000:
- $15 for a typical 30-day prescription
- $22.50 for a typical 60-day prescription
- $30 for a typical 90-day prescription
Individuals with annual incomes between $15,001 and $30,000:
- $25 for a typical 30-day prescription
- $37.50 for a typical 60-day prescription
- $50 for a typical 90-day prescription

The AZ&Me™ Prescription Savings program for people with Medicare Part D is not part of the Medicare program, but is an independent assistance program provided by AstraZeneca designed to help people with limited income.
This is just one of the patient assistance offerings that provide AstraZeneca medicines at significant savings or free of charge to individuals.
